Policy for management of personal information

This document describes the policy of Sally Hanratty Psychology for the management of the practice’s client’s information.  The psychological service provided is bounded by the legal requirements f the Australian Privacy Principles (2014) (APP) and the Victorian Health Privacy Principles(HPP).

Client information

Client files are held in a secure online file that is only accessible to Sally Hanratty. The information on each file includes personal information such as name, address, contact phone numbers and other information relevant to the psychological service being provided.

Purpose of holding the information

The information is gathered and kept as part of the assessment and diagnosis of the client and is only seen by the psychologist and the Autism Care Team with appropriate consent.  In addition, information needs to be retained to meet the legal requirements of the Australian Privacy Principle and Victorian Health Privacy Principle. 

Requests for access to client information

Clients may request to see the information about them kept on file.  The psychologist may discuss the contents with them or provide a copy.  All requests to access information should be directed to Sally Hanratty Psychology.  These requests will be responded to within 14 days and an appointment made if necessary for clarification purposes.

Concerns

If you have any concerns about the management of your personal information, please inform Sally Hanratty.  Upon request, you may obtain a copy of the Australian Privacy Principles, which describe your rights and how your information should be handled. 

Ultimately, if you wish to lodge a formal complaint about the use of, or access to, your personal information, you may do so either with the Office of the Australian Information Commissioner on 1300 363 992 or the Health Complaints Commissioner on 1300 582 113.
